Nurse Practitioner (Remote, IL License Required) (Medtech)

Location: Remote, United States; active Illinois licensure required

Salary: $75/hour regular rate; $85/hour premium rate

What you will do

• Provide phone- and video-based follow-up care for patients after hospital discharge.
• Conduct outreach, including cold calls, and scheduled virtual visits.
• Perform medication reconciliation and symptom screening, and order follow-up labs, medications, and supplies.
• Build patient trust, explain the care model, and connect patients with appropriate services.
• Coordinate care across hospitals, emergency departments, pharmacies, and community providers.
• Collaborate with nursing and administrative colleagues while contributing to individual and population health goals.

Requirements

• Certified Nurse Practitioner specializing in Adult, Family, or Geriatric care.
• Active Illinois state licensure required, with the ability to obtain additional state licenses.
• 5+ years of post-graduate clinical experience in primary care or geriatrics; up to two years may be substituted with skilled-nursing or long-term-care experience.
• Experience caring for Medicare and Medicaid populations with complex, comorbid conditions.
• Motivational interviewing, strong written and verbal communication, collaboration, and problem-solving skills.
• Availability for at least 24 hours per week, including monthly weekend coverage and four-hour shifts across weekdays and weekends.

Nice to have

• Active licenses in Texas, Pennsylvania, New Jersey, New York, Massachusetts, Minnesota, Louisiana, or South Carolina.
• Bilingual English and Spanish communication skills with cultural fluency.
• Experience with virtual care and familiarity with Google Suite and Slack.
