Nurse Practitioner

Location: On-site in Everett, Washington, United States, with in-home visits across Oregon

Salary: Approximately $140,000 annually

What you will do

• Travel to members’ homes and conduct comprehensive wellness, health, and risk adjustment assessments.
• Perform medical histories, physical examinations, vital sign collection, medication reconciliation, and preventive screenings.
• Educate patients on prevention, medication adherence, chronic condition management, and goals of care.
• Identify diagnoses and quality care gaps, and provide findings for post-visit summaries and care management.
• Collaborate with primary care and broader care teams to improve clinical quality and patient experience.
• Maintain accurate, timely, HIPAA-compliant EMR documentation and protect patient health information.

Requirements

• Active, unencumbered Nurse Practitioner license in Oregon required.
• At least three years of direct patient care experience; primary care or adult/geriatric experience is preferred.
• Ability to provide in-home care and travel within a 55-mile radius of the assigned home location.
• Ability to support statewide Oregon travel with at least two weeks’ notice.
• EMR proficiency is desired.
• Bilingual proficiency is advantageous.

• Prior in-home risk assessment experience.
• Primary care or adult/geriatric clinical background.
• Bilingual proficiency.
