Care Coach – Seasonal Work-From-Home Opportunity (Healthcare)

What you will do

• Conduct health assessments and collect relevant patient information.
• Help adult patients understand healthcare benefits, services, and available resources.
• Support care coordination and connect patients with appropriate services.
• Communicate with healthcare professionals to support continuity of care.
• Educate and guide patients, including elderly individuals and people with chronic conditions.
• Document patient interactions accurately in electronic systems and build trusted relationships with patients and stakeholders.

Requirements

• Associate degree or higher in Allied Sciences, Social Sciences, Social Work, Human Services, a related field, or any discipline.
• At least 1 year of direct adult patient care experience with an allied or social science degree, or at least 2 years with a general degree.
• Experience may come from long-term care, home or community-based services, hospitals, healthcare facilities, case management, managed care, or similar environments.
• Strong communication, rapport-building, compassion, critical-thinking, problem-solving, and documentation skills.
• Proficiency with Microsoft Word, Excel, Outlook, and electronic record systems.
• Must work from home in Wyoming, United States. Nursing and physician degrees do not meet the educational profile requirements.

Nice to have

• Experience supporting elderly or chronically ill populations, Medicare or Medicaid members, or patients with chronic conditions.
• Experience in care coordination, case management, managed care, health assessments, patient education, telephonic health coaching, or remote care management.
• Experience working with multidisciplinary healthcare teams or in healthcare business process outsourcing.
