Onsite Care Coordination Nurse (Healthcare)

Location: Hybrid/onsite at Baptist Health Deaconess, 900 Hospital Drive, Madisonville, Kentucky. Must live within a reasonable commute, preferably 30–60 minutes driving distance from the facility. Limited remote work is available for coordination and documentation.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support discharge planning from admission through post-discharge and facilitate follow-up with primary care providers.
• Identify care gaps, safety concerns, and barriers to discharge, then implement strategies to address them.
• Collaborate with attending providers, facility case managers, and other healthcare professionals to coordinate care.
• Educate members and families about diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation accurately and follow infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Registered Nurse license in Kentucky.
• Must live within a 30–60 minute driving distance of Baptist Health Deaconess in Madisonville, Kentucky.
• At least three years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, personal vehicle liability insurance, required screenings, and a private workspace for remote documentation.

Nice to have

• Bachelor’s degree in Nursing.
• Experience in care management, health promotion, coaching, utilization management, or health plans.
• Care management or related specialty certification.
• Knowledge of HEDIS and Stars measures.
• Bilingual English/Spanish skills.
