Senior Care Specialist (Registered Nurse) – Work From Home

Location: Work from home in Ohio, United States

Starting pay: $37.00 per hour; pay may be negotiable based on experience, qualifications, skills, and business requirements.

What you will do

• Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members.
• Develop individualized, measurable care plans and provide chronic-condition education, coaching, and support.
• Use clinical judgment to assess needs, coordinate interventions, and address physical, environmental, and psychosocial barriers to care.
• Coordinate transitions of care, safe discharge planning, durable medical equipment, home health services, provider appointments, and other resources.
• Collaborate with members, caregivers, providers, social workers, dietitians, pharmacists, and multidisciplinary care teams.
• Document assessments, care plans, interventions, member interactions, and outcomes accurately and promptly in applicable systems.

Requirements

• Active, unrestricted Registered Nurse license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• At least 2 years of clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment, critical-thinking, problem-solving, verbal, and written communication skills.
• Ability to work independently in a remote environment and navigate multiple computer systems and electronic medical records.
• Intermediate to advanced proficiency with Microsoft Word, Excel, and Outlook.

Nice to have

• Experience caring for elderly or chronically ill populations.
• Experience in care coordination, case management, managed care, Medicare, or Medicaid.
• Experience with chronic condition management, patient education, telephonic nursing, health coaching, or remote care management.
• Experience working with multidisciplinary healthcare teams.
