Onsite Care Coordination Nurse (Acute Care)

Location: Hybrid/onsite at Wellstar MCG Health Medical Center, 1120 15th Street, Augusta, Georgia. Must live within a reasonable commute, preferably within 30–60 minutes by car. Limited documentation and coordination work may be completed remotely from a private workspace.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support members and families with discharge planning, education, and post-discharge primary care follow-up.
• Identify care gaps, safety concerns, and discharge barriers, then implement strategies to address them.
• Coordinate care with attending providers, facility case managers, and other healthcare professionals.
• Educate members about diagnoses, medications, and preventive care to reduce readmission risk.
• Complete documentation while maintaining infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Registered Nurse license in Georgia.
• Must live within a 30–60 minute driving distance of the assigned Augusta facility.
• At least three years of recent acute care nursing experience.
• Experience with hospital or health plan discharge planning or care coordination.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, personal vehicle liability insurance, TB screening, and required background and facility credentialing clearances.

Nice to have

• Bachelor’s degree in Nursing.
• Care management, health promotion, coaching, utilization management, or health plan experience.
• Bilingual English/Spanish skills.
• Care management or related specialty certification.
• Knowledge of HEDIS and Stars measures.
