Care Manager Transition of Care (RN)

Location: Remote in California; candidates must hold an active California RN license. Preference is given to candidates residing in California and able to work Pacific Time (PST) hours.

Salary: $27.02–$48.55 per hour

What you will do

• Complete post-discharge assessments for members transitioning from healthcare facilities.
• Review and reconcile pre-admission and post-discharge medications.
• Develop care and service plans with discharge planners, providers, specialists, and interdisciplinary teams.
• Assess health status, treatment plans, resource needs, and required interventions.
• Facilitate transitions into active care management and coordinate medical and non-medical services.
• Provide education, resource materials, outreach, documentation, and feedback on care quality and cost effectiveness.

Requirements

• Active RN license in California required; other listed qualifying licenses include LISW, LCSW, LMSW, LMFT, LMHC, and LPC.
• Degree from an accredited school of nursing, or a master's degree in Behavioral Health or Social Work.
• 2–4 years of related experience.
• Experience in case management, transitional care, care coordination, or discharge planning is strongly preferred.
• Ability to work Pacific Time hours preferred.
