Manager, Advanced Practice Provider - Transitions of Care

Location: Remote USA; Monday-Friday, 9:30am-6:00pm EST

Salary: $121,000-$155,000 annually

What you will do

• Provide care to patients during the 30-day post-discharge period, identify gaps in care, escalate symptomatic patients, and reduce preventable readmissions.
• Oversee clinical performance and accountability for APPs in the Transitions of Care program.
• Manage performance, set goals, conduct bi-weekly one-on-ones, and support the development of direct reports.
• Develop and track clinical and operational KPIs, identify improvement opportunities, and implement strategies to achieve target metrics.
• Collaborate with medical directors, APP managers, operations leaders, and education teams to improve workflows, policies, onboarding, and clinical processes.
• Support recruitment, hiring, scheduling oversight, resource allocation, and communication of APP organization needs to TOC leadership.

Requirements

• 5+ years of clinical practice experience as an Advanced Practice Provider, Nurse Practitioner, or Physician Assistant.
• At least 1 year of experience caring for post-discharge patients and 2+ years of management or team leadership experience.
• Active, unrestricted RN and APRN license or Physician Assistant license in the home state, national APRN certification, and/or the ability to obtain licenses in multiple states.
• Availability to work Monday-Friday from 9:30am-6:00pm EST.
• Experience with telehealth visits, post-transition patient management, acute and chronic disease exacerbations, and identifying the need for immediate intervention.
• Ability to collaborate on process improvement initiatives and apply PDSA principles.

• Experience managing CHF exacerbations, diabetic emergencies, COPD exacerbations, and hypertensive emergencies.
