Bilingual RN Care Coordinator (Spanish)

Location: Remote within the Eastern or Central time zones; business hours are weekdays 9–5 CST. A dedicated private workspace, secure handling of sensitive documents, and high-speed internet are required.

Salary: $80,000–$90,000 per year

What you will do

• Conduct proactive and reactive member outreach using risk stratification and care gap data.
• Complete clinical, psychosocial, and social determinants of health assessments and develop individualized care plans.
• Coordinate services and resources across the care continuum while monitoring member progress.
• Educate members and providers about benefits, digital health solutions, and evidence-based self-management.
• Support utilization review by coordinating communication among members, providers, and the utilization management team.
• Document activities, manage escalations, and promote enrollment in digital health programs.

Requirements

• Diploma from an accredited nursing school; an ADN or higher is preferred.
• Current, unrestricted Texas RN license or active Compact (NLC) nursing license required.
• 3–5 years of direct patient care nursing experience plus 1–2 years in a health plan or managed care organization.
• Working knowledge of health plan benefits, CPT/ICD-10 codes, authorizations, and pre-certification processes.
• Fluency in English and Spanish required.
• Must reside in the Eastern or Central time zones.

Nice to have

• BSN or higher.
• Certified Case Manager (CCM) or American Case Management (ACM) certification.
• Experience in telephonic care coordination, outreach nursing, or call center-based patient engagement.
• Experience with HEDIS measures, care gap closure, or utilization management, including MCG or InterQual guidelines.
