TL;DR
Nurse Practitioner (Medicare): Conducting in-home wellness and risk adjustment assessments for Medicare and other patient populations with an accent on comprehensive examinations, preventive screenings, chronic condition management, and compliant clinical documentation. Focus on traveling to members’ homes, identifying care gaps and clinically relevant diagnoses, and coordinating findings with primary care providers across Oregon.

Location: Hood River, Oregon, United States; on-site. Full-time providers support the entire state of Oregon, with daily travel within a 55-mile radius of the assigned home location and scheduled statewide travel with at least two weeks’ notice.

Salary: approximately $130,000 annually, plus sign-on bonus.

What you will do

  • Conduct in-home Annual Wellness Visits and comprehensive health and risk adjustment assessments.
  • Perform medical histories, physical examinations, review of systems, vital signs collection, and medication reconciliation.
  • Complete applicable preventive screenings, including Hemoglobin A1C, spirometry, KED, DRE, FIT, and bone density assessments.
  • Provide patient education on preventive care, medication adherence, chronic condition management, and goals of care.
  • Identify diagnoses and care gaps, document findings in the EMR, and support follow-up care.
  • Collaborate with primary care providers and the broader care team while safeguarding protected health information.

Requirements

  • Active, unencumbered Nurse Practitioner license in Oregon.
  • At least 3 years of direct patient care experience; primary care or adult/geriatric experience is preferred.
  • Ability to travel to members’ homes throughout Oregon and work within the stated travel radius.
  • Proficiency with EMR systems is desired.
  • Ability to maintain accurate, timely, and compliant clinical documentation and follow HIPAA requirements.

  • Prior in-home risk assessment experience.
  • Bilingual proficiency.
