Care Coordinator II (Healthcare)

Location: Remote-AR; must reside in Arkansas or a border city and support members throughout Washington and Benton counties, with additional travel potentially required across Madison, Crawford, Franklin, and Sebastian counties. Field-based work includes home visits and 30% travel.

Salary: $17.84–$28.02 per hour

What you will do

• Conduct member outreach by phone and through home visits to discuss care plans, services, resources, concerns, and next steps.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and internal care teams.
• Support member and provider inquiries and help maintain continuity of care and service.
• Perform service assessments and screenings, identify needs, and make referrals to care managers, disease managers, and community-based organizations.
• Document and maintain member records in accordance with state and regulatory requirements.
• Provide education about available benefits and resources while following contractual and regulatory standards.

Requirements

• High school diploma or GED and 1–2 years of related experience.
• For the Arkansas Total Care plan: a bachelor's degree in social science or a health-related field, or a high school diploma with at least one year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health needs.
• Strong experience working with the developmental disabilities population is preferred for this field-based role.
• Must reside in Arkansas or a border city and be able to support members across the specified Arkansas counties.
• A valid driver's license, child and adult maltreatment checks, and drug screening are required before hire and on a recurring basis.
• Travel: 30% required.
