Outreach Specialist

What you will do

• Provide telephonic and virtual outreach and engagement services to eligible members.
• Use Commons data to understand potential members and explain the outreach process.
• Review and update demographic information and emergency contacts in Commons.
• Collect verbal and written consent and initiate the member intake process.
• Communicate urgent needs and relevant information to care teams and coordinate member handoffs.
• Track tasks, organize information, monitor performance, communicate with members and families, and provide feedback to product development.

Requirements

• At least 1 year of experience in a similar role.
• Must be based in North Carolina, United States, for this remote role.
• Ability to conduct telephonic and virtual member outreach.
• Ability to document accurate information, collect consent, and communicate effectively with members, families, and care teams.
