Community Health Worker (Healthcare)

Location: Virtual full-time work is available only for candidates within a reasonable commuting distance of a posting location in Florida, Nevada, Kansas, Virginia, Tennessee, Colorado, Missouri, or Texas; in-person training and travel to worksites or other locations may be required. Work hours follow Pacific or Mountain Time, with an 8-hour shift between 8:00 a.m. and 6:00 p.m.

Salary: $17.36–$31.52 per hour for positions in Colorado, Nevada, and Virginia.

What you will do

• Serve as a liaison between patients, care teams, families, and community organizations.
• Conduct telephonic outreach to assess needs, schedule appointments, and close care gaps.
• Identify social determinants of health and collaborate with social workers or care managers on action plans.
• Build trusting relationships through patient support, advocacy, and assistance with healthcare and community-resource access.
• Facilitate communication, help with enrollment forms and specialist appointments, and document interactions in medical records.
• Attend required meetings and travel to worksites or other locations as needed.

Requirements

• High school diploma or equivalent.
• At least one year of experience in healthcare, a community-based environment, or social work, or an equivalent combination of education and experience.
• Experience using electronic medical records.
• Ability to work within the stated U.S. location and commuting-distance requirements, including required in-person training.
• Availability for Pacific or Mountain Time work hours.

Nice to have

• Bilingual communication skills.
• Certified Community Health Worker credential.
