Community Health Worker - Medicaid

Location: Remote from Pennsylvania, United States; travel required 50%–75%.

What you will do

• Conduct face-to-face and telephone outreach to Medicaid members regarding preventive care, care gaps, maternity, care transitions, chronic conditions, and health-related social needs.
• Educate members on preventive health services, health plan benefits, and healthcare navigation.
• Assess social needs, coordinate referrals, schedule appointments, and connect members with community resources.
• Build partnerships with providers, community organizations, Head Start programs, WIC, community centers, and homeless shelters.
• Prepare educational materials, document outreach outcomes, and support programs focused on at-risk members.
• Represent the organization at community events and collaborate with care management and internal teams.

Requirements

• High school diploma or GED.
• At least one year of experience in healthcare-related customer service or marketing and one year of Community Health Worker or related experience.
• Required certification: Certified Community Health Worker (CCHW) from the PA Certification Board within 12 months of the start date.
• Strong written and verbal communication, customer service, problem-solving, organization, patience, and empathy.
• Ability to work sensitively with Medicaid members and people with limited English proficiency.
• Must work from Pennsylvania and be able to travel 50%–75%.
