Case Manager - Behavioral Health

Location: Hingham, Massachusetts, United States

Hourly range: $40.29–$49.25 per hour, plus eligibility for variable pay.

What you will do

• Assess member and family needs, health disparities, social determinants of health, and barriers to care.
• Develop, document, and monitor comprehensive care management plans with measurable goals.
• Support treatment-plan adherence, shared decision-making, medication education, and complex pharmacy needs.
• Coordinate care transitions and collaborate with physicians, therapists, treatment providers, facilities, and internal clinical teams.
• Monitor clinical quality concerns, make referrals, escalate issues, and present appropriate cases at medical rounds.
• Educate members and provide resources while supporting culturally and demographically diverse populations.

Requirements

• Active Massachusetts licensure is required as an RN, LMHC, LICSW, or LCSW; behavioral health candidates must hold one of these licenses.
• 3–5 years of relevant clinical healthcare experience in inpatient, outpatient, or other care settings.
• Ability to assess, analyze, interpret, and document complex medical information and actionable member goals.
• Strong written and verbal communication, critical-thinking, collaboration, organization, and deadline-management skills.
• Proficiency with multiple IT systems and the ability to learn new clinical and business skills.
• For registered nurses, a Bachelor’s degree in nursing is preferred; additional state licensure is a plus.

Nice to have

• Case Management experience.
• Case Management, certified diabetic educator, or Wellcoach certification.
• Licensure in additional states.
• BSN for registered nurse candidates.
