LTSS Service Care Manager

Location: Field-based role; applicants must reside in Fort Myers, Bonita Springs, and/or Estero, Florida. Monday through Friday, 8:00 AM to 5:00 PM.

Pay range: $27.02–$48.55 per hour.

What you will do

• Evaluate member needs, available resources, and appropriate care outcomes.
• Develop and monitor long-term care plans and service plans.
• Coordinate services among members, families, caregivers, providers, specialists, and community resources.
• Provide resource support and education on healthcare benefits, service options, referrals, and provider instructions.
• Document member information and care management activities in compliance with regulatory and payer requirements.
• Conduct home or other site visits when needed and provide feedback on quality and service improvements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or an applicable equivalent combination of education and experience.
• Alternative qualifications include a relevant bachelor’s degree with at least one year of qualifying experience, or a high school diploma with three years of qualifying case-management experience.
• Experience may involve care management for older adults, behavioral health conditions, physical or developmental disabilities, or people living with HIV/AIDS.
• Strong assessment, problem-solving, time-management, customer service, and communication skills.
• Must reside in Fort Myers, Bonita Springs, and/or Estero, Florida.

Nice to have

• Bilingual Spanish and English proficiency.
• Experience in case management and strong technology skills.
