Connected Care Coach (Healthcare)

Location: USA - Any Location (Remote)

Salary: $24–$26 per hour, plus performance bonus and eligible benefits.

What you will do

• Conduct proactive outreach to identified members through phone, email, text, virtual applications, and coaching conversations.
• Use motivational interviewing techniques to support behavior change and chronic condition self-management.
• Provide feedback, answer questions, share resources, and hold members accountable to their health goals.
• Refer members to condition-specific support and education in line with clinical care guidelines.
• Maintain accurate case documentation and complete administrative duties.
• Support corporate initiatives and special projects.

Requirements

• Bachelor’s degree in a relevant discipline, or MA/EMT/Paramedic certification.
• At least 2 years of experience in a healthcare delivery or related environment.
• Excellent written and verbal English communication skills.
• Strong customer service, interpersonal, organizational, analytical, and critical-thinking skills.
• Ability to work autonomously in a fast-paced environment and use multiple technology platforms, including Microsoft Office, Teams, SharePoint, OneDrive, Salesforce, and Jira.
• Must be currently authorized to work in the United States without visa sponsorship now or in the future.
