Care Manager, Behavioral Health

Location: Remote Illinois; must reside in Illinois or a border state. The role requires up to 25% travel to facilities, homes, offices, and community locations for face-to-face assessments, collaboration, training, and meetings.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct face-to-face and telephonic behavioral health assessments, including Comprehensive Risk Assessments.
• Coordinate integrated behavioral and medical services for members with serious mental illness, serious emotional disturbance, and substance use disorders.
• Develop care plans, oversee transitions and service delivery, and facilitate interdisciplinary care team meetings.
• Provide crisis intervention, peer support, health literacy support, and referrals to community health and social service resources.
• Engage members in their communities and conduct recurring face-to-face visits to monitor progress and address barriers to care.
• Collaborate with providers, internal departments, service coordinators, and community organizations while maintaining compliance with IDHS, CMS, and NCQA requirements.

Requirements

• Active Illinois license as an LCSW, LMFT, or LCPC; provisional licenses are not accepted.
• Must reside in Illinois or a border state.
• At least two years of post-degree clinical experience in a behavioral health setting.
• Case management experience with complex SMI, SUD, and SED populations.
• Ability to travel to region-based facilities and members' homes for face-to-face assessments.
• Valid state driver's license and personal vehicle liability insurance with limits of at least 100,000/300,000/100,000; TB screening is required.

Nice to have

• Case Management Certification (CCM).
• Three or more years of care coordination experience, including in-home assessments.
• Experience with Medicaid populations, managed care, or field case management.
• Knowledge of community health and social service agencies.
• Bilingual skills.
