RN Care Manager Lead

Location: Remote Wisconsin; must reside within a commutable distance of the Family Care Partnership service area in Southeast Wisconsin. Field work includes travel of approximately 20% throughout Kenosha, Racine, Milwaukee, Waukesha, Walworth, Washington, and Sheboygan counties. Work hours are Monday–Friday, 8:30 a.m.–5:00 p.m. Central Standard Time.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide lead care management services within the Family Care Partnership program.
• Mentor care managers, nurses, and care coaches through orientation, coaching, training, and field shadowing.
• Conduct nursing assessments and periodic reassessments, update care plans, and monitor member health and safety.
• Coordinate medical, behavioral health, social, rehabilitation, vocational, and educational services.
• Use Resource Allocation Decision-making methodology to monitor services, resources, and member outcomes.
• Collaborate with the Interdisciplinary Team and leadership on contractual compliance, best practices, and training effectiveness.

Requirements

• Registered Nurse licensed and in good standing in Wisconsin.
• At least 2 years of managed care experience.
• Residence within a commutable distance of the Southeast Wisconsin Family Care Partnership service area.
• Ability to travel up to 20% throughout the service area.
• Valid state driver's license and personal vehicle liability insurance with limits of at least 100/300/100.
• Ability to meet patient-facing TB screening requirements and protect member PHI/HIPAA information while working from home.

Nice to have

• English and Spanish bilingual skills.
• Training or mentoring experience.
• Knowledge of community health, social service agencies, and community resources.
