Senior Case Manager

Location: Mendota Heights, Minnesota, USA

Salary: $55,100–$56,615.25 annually, based on experience, skills, and qualifications.

What you will do

• Coordinate services and supports for vulnerable individuals living in the community.
• Conduct ongoing assessment, planning, referrals, monitoring, consultation, and advocacy.
• Manage a direct caseload and coordinate multiple service providers and support systems.
• Help individuals and guardians make informed choices about services and supports.
• Act as an educator and resource for clients, families, peers, and community organizations.
• Drive as part of the role, including transporting people served when required.

Requirements

• Bachelor’s degree in a human service-related field.
• Two years of experience working with people with disabilities or older adults.
• Two years of full-time experience in CADI/DD waiver case management.
• Knowledge of the local community and self-directed services.
• Strong oral and written communication skills.
• Valid driver’s license, automobile insurance, and eligibility for an unlimited driving position are required.
