Case Manager- MKE

Location: Remote position serving Milwaukee County and surrounding areas, Wisconsin; up to 40% travel and occasional travel to hirify.global offices may be required.

Salary: $53,700–$72,600 per year

What you will do

• Assess members’ health and safety needs, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and service providers to deliver support in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at members’ residences.
• Complete quarterly in-person visits and maintain monthly telephone contact with members.
• Arrange community, housing, employment, social integration, and other non-medical support services.
• Evaluate risks, educate members, authorize services, and maintain accurate case documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum limits of 100/300/100 are required.
• Ability to serve members in Milwaukee County and surrounding areas and complete recurring in-person visits.
• Availability to work Monday–Friday, 8:00 a.m.–4:30 p.m. CST.
• Ability to meet patient-facing TB screening requirements and protect member PHI/HIPAA information when working remotely.

Nice to have

• Residence within 45 minutes of the assigned coverage area.
• Case management experience.
• Experience with electronic case-note documentation.
• Knowledge of community health, social service agencies, and other community resources.
