Case Manager, Health Homes (Buffalo, NY / Field Based) (Healthcare)

Location: Field-based in Buffalo, NY and surrounding areas within Erie County; conditions listed as Chautauqua, NY. Frequent travel to patients’ homes and in-person patient meetings are required.

Salary: $24.00–$26.00 per hour. A $500 sign-on bonus is paid after successfully completing six months of employment.

What you will do

• Conduct outreach, screen eligibility, obtain consents, and enroll patients in the Health Home Care Management Program.
• Complete initial and ongoing assessments covering medical, mental health, substance use, financial, housing, and support needs.
• Develop, implement, and document patient-centered care plans for patients with chronic conditions.
• Coordinate services with medical providers, pharmacies, insurers, hospitals, family caregivers, and community organizations.
• Facilitate referrals, address access barriers, connect patients with local resources, and support care-plan goals.
• Conduct home visits, provide crisis intervention when needed, maintain patient contact, and document services in accordance with Health Homes standards.

Requirements

• At least one year of Health Homes, HARP, or case management experience.
• Associate’s degree minimum; a bachelor’s degree is preferred.
• Ability to travel in the field, accompany patients to appointments, and meet patients in person.
• Strong communication, assessment, empathy, documentation, data reporting, and caseload-management skills.
• Knowledge of local community resources and experience handling rapidly changing crisis situations.
• Experience using multiple data management systems, such as Salesforce.

Nice to have

• Strongly preferred bilingual Spanish skills.
• Master’s degree.
• Three or more years of Health Homes or HARP experience.
