Case Manager (Substance Use Disorder)

Location: On-site at 82 Pond Street, Pawtucket, Rhode Island, USA

Pay: $21 per hour plus a bonus structure

What you will do

• Provide individualized case management for patients with substance use disorders throughout their recovery journey.
• Orient patients to treatment services, policies, and expectations.
• Coordinate internal and community referrals, follow-up, and continuity of care.
• Assess social, educational, vocational, and recovery needs and maintain individualized treatment plans.
• Provide crisis intervention and support during critical moments.
• Support program implementation, process improvements, caseload management, and community outreach.

Requirements

• Bachelor’s degree or higher in Human Services or a related field.
• Ability to meet applicable federal and state certification and licensure requirements.
• Availability Monday through Friday from 5:30 a.m. to 2:00 p.m., with a rotating Saturday shift.
• Ability to work in a fast-paced clinical environment involving prolonged standing, bending, keyboarding, and computer use.

Nice to have

• Certification or pursuit of certification as a Certified Alcohol and Drug Counselor (CADC).
• Knowledge of addiction treatment methods and opioid maintenance treatment trends.
