Case Manager- Dane/Columbia (Wisconsin Family Care): Providing comprehensive case management for frail elders and adults with intellectual, developmental, or physical disabilities with an accent on health and safety assessments, member care plans, and coordinated social support. Focus on conducting recurring in-person member visits, arranging community services, evaluating risks, and maintaining accurate case documentation.
Location: Remote within the Dane and Columbia counties and surrounding area in Wisconsin, United States; up to 40% travel for member visits and occasional office training or meetings. Work hours are Monday–Friday, 8:00 am–4:30 pm CST.
Salary: $53,700–$72,600 per year

What you will do
- Assess members’ health and safety needs, strengths, interests, and preferences.
- Develop and update comprehensive Member Care Plans.
- Coordinate services with Field Care Nurses and other providers in the least restrictive environment.
- Conduct face-to-face social assessments at enrollment and at least every six months, plus quarterly in-person visits and monthly phone contact.
- Arrange social integration, community resource, employment, housing, and other non-medical support services.
- Evaluate risks, educate members, ensure cost-effective service delivery, and maintain case documentation and service authorizations.
Requirements
- Bachelor’s degree in human services or a related field plus at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least 3 years of relevant experience.
- Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
- Ability to serve members in the assigned Wisconsin coverage area and travel up to 40%.
- Ability to work patient-facing and in members’ residences, with TB screening required.
- Ability to maintain a dedicated, interruption-free workspace and internet service with at least 25 Mbps download and 10 Mbps upload for protected member information.
Nice to have
- Residence within 45 minutes of the assigned coverage area.
- Case management experience.
- Experience with electronic case note documentation.
- Knowledge of community health, social service agencies, and community resources.
