Advanced Practice Provider: Complex Geriatric Care (Healthcare)

Location: Remote USA; working hours must align with EST or CST. An active and clear RN and APRN or Physician Assistant license in GA, PA, and/or OH is required at hire.

Salary: $120,000–$155,000 annual base salary, plus a performance-based bonus of $10,000–$20,000, for a total compensation range of $130,000–$170,000.

What you will do

• Deliver video telehealth care to a cohort of complex patients across multiple geographies.
• Guide and oversee medical management using the 5M framework, including symptom and chronic disease management.
• Coordinate care with primary care providers, specialists, pharmacy, family members, caregivers, and other support teams.
• Conduct comprehensive assessments, diagnosis and disease reviews, medication reviews, and post-acute transition support.
• Assess quality-of-care interventions, social needs, home health requirements, and durable medical equipment needs.
• Maintain accurate medical records and comply with legal, ethical, privacy, and telehealth regulations; occasional weekend on-call support may be required.

Requirements

• Active and clear RN and APRN or Physician Assistant license in GA, PA, and/or OH at the time of hire.
• Willingness to obtain and maintain multiple state licenses.
• Three or more years of APP experience in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care.
• Proficiency with telehealth technology and electronic health records.
• Active BLS certification at hire and throughout employment.

Nice to have

• Virtual care experience and interest in continuing clinical nursing through virtual visits.
• Experience collaborating with interdisciplinary teams and caring for complex, frail, or vulnerable populations, including in an SNF setting.
• Spanish proficiency, a continuous improvement mindset, adaptability, and experience in a fast-paced organization.
