Lead Care Manager (RN)

Location: Remote-FL; remote work in the United States

Salary: $87,000–$161,300 per year

What you will do

• Oversee and review personalized care plans and service plans for complex-care members.
• Collaborate with healthcare providers, specialists, and community resources to address unmet member needs.
• Monitor member conditions, outcomes, care gaps, and health risks, revising care plans when needed.
• Provide clinical guidance, quality control, training, and subject matter expertise to care management team members and providers.
• Handle escalated and complex cases with senior management and support junior team members.
• Conduct telephonic, digital, home, and other site outreach in an inbound/outbound call center environment.

Requirements

• Degree from an accredited school of nursing or a Bachelor's degree in Nursing.
• 5–7 years of related experience.
• Registered Nurse state licensure and/or compact state licensure required.
• Experience working with care plans, healthcare providers, member needs, and regulatory requirements.
• Must be eligible to work as an RN in the relevant US state or compact-state jurisdiction.

Nice to have

• Specialty pharmacy experience.
