What you will do

  • Lead and support a virtual team of Transitions of Care Coordinators serving patients transitioning home from hospitals or post-acute settings.
  • Provide direct transitions-of-care patient support, including coordination of appointments, transportation, supplies, durable medical equipment, and follow-up communication.
  • Monitor team performance, attendance, adherence, meetings, communication channels, and one-on-one coaching.
  • Develop and implement workflows, job aids, process improvements, and training initiatives.
  • Support recruiting, onboarding, clinical model pilots, and cross-functional meetings with TOC leadership and partner teams.
  • Review team performance with the Senior Manager and recommend improvements.

Requirements

  • Two or more years of direct lead or supervisory experience in a virtual setting.
  • Medical Assistant certification is desired; active or expired certifications are accepted.
  • Candidates without certification may provide significant Medical Assistant experience and, if requested, school diploma and transcript documentation.
  • Two or more years of healthcare administrative or office experience is desired.
  • Strong organization, attention to detail, and ability to work with evolving technology platforms.

  • Fully bilingual English/Spanish communication skills.
