Onsite Care Coordination Nurse

Location: Hybrid/onsite at Atrium Health Union, 600 Hospital Drive, Monroe, North Carolina. Must live within a reasonable commute, preferably 30–60 minutes by car. Limited documentation work may be completed remotely at the leader’s discretion.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support members and families with discharge planning from admission through post-discharge follow-up.
• Identify care gaps, safety concerns, and discharge barriers, then implement appropriate interventions.
• Coordinate care with attending providers, facility case managers, and other healthcare professionals.
• Educate members about diagnoses, medications, preventive measures, and ongoing wellness.
• Complete documentation accurately and follow infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Registered Nurse license in North Carolina.
• Must live within a 30–60 minute driving distance of Atrium Health Union in Monroe, North Carolina.
• At least 3 years of recent acute care nursing experience, such as medical-surgical, critical care, heart/lung, skilled nursing, or rehabilitation.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, personal vehicle liability insurance, required screenings, and a private workspace for remote documentation.

Nice to have

• Bachelor’s degree in Nursing.
• Experience in care management, health promotion, coaching, utilization management, or health plans.
• Bilingual English/Spanish skills.
• Care management or related specialty certification.
• Knowledge of HEDIS and Stars measures.
