LTSS Service Care Coordinator

Location: Remote, Texas; must reside in the state of Texas. The role is primarily a remote call center position and may include on-site member visits.

Salary: $22.94–$38.79 per hour

What you will do

• Assess member needs, available resources, and appropriate care outcomes.
• Develop and monitor long-term care plans and service plans.
• Coordinate services among members, families, caregivers, healthcare providers, specialists, and community resources.
• Provide support with housing, employment, independent living, foster care, justice, and other member needs.
• Educate members and caregivers about healthcare benefits, procedures, referrals, service options, and provider instructions.
• Document care management activities, monitor outcomes, support regulatory compliance, and recommend quality improvements.

Requirements

• Bachelor’s degree and one year of related experience, or equivalent applicable experience.
• Direct experience within three of the last five years working with people with disabilities or vulnerable populations with chronic or complex conditions, including children and young adults.
• Must reside in Texas.
• Knowledge of Medicare and Medicaid programs is advantageous.
