Field Care Manager, Behavioral Health

Location: Remote Illinois; applicants must reside in Cook or Will County, IL, within the listed ZIP codes or within a 10-mile radius. The role includes 75–90% local travel to members’ homes and care facilities.

Salary: $71,500–$97,500 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ behavioral health needs and establish individualized care plans.
• Plan and implement interventions to support members’ wellness and desired outcomes.
• Coordinate services with providers, payer sources, interdisciplinary teams, families, and community resources.
• Monitor and evaluate care plans through assessments, data, conversations, and active care planning.
• Conduct face-to-face assessments in homes, assisted living facilities, long-term care facilities, and other care settings.
• Identify and resolve barriers to effective care while guiding members and families toward appropriate resources.

Requirements

• Must reside in Cook or Will County, Illinois, within the specified ZIP codes or within a 10-mile radius.
• Active Illinois license as an LCSW, LMFT, or LCPC; supervisee and provisional licenses are not accepted.
• At least 2 years of post-degree clinical experience in a behavioral health setting.
• Case management experience with complex SMI, SUD, and SED populations.
• Ability to travel to regional facilities and homes for face-to-face assessments.
• Valid driver’s license, vehicle insurance, reliable transportation, electronic medical record experience, and intermediate to advanced computer skills.

Nice to have

• Case Management Certification (CCM).
• At least 3 years of in-home assessment or care coordination experience.
• Experience with Medicare, Medicaid, dual-eligible populations, field case management, managed care, and community health or social service agencies.
• Bilingual ability.
