Senior Care Specialist (Registered Nurse) – Work From Home

Location: Work from home in the USA

Starting pay: $37.00 per hour; pay may be negotiable based on relevant experience, qualifications, skills, and business requirements.

What you will do

• Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members.
• Develop individualized, measurable care plans based on member needs and health risks.
• Build health-coaching relationships and provide education, encouragement, and chronic-condition management support.
• Use clinical nursing judgment to coordinate interventions for acute situations and transitions of care.
• Coordinate durable medical equipment, home health services, provider appointments, benefits, and other care resources.
• Collaborate with members, caregivers, providers, social workers, dietitians, pharmacists, and multidisciplinary care teams while documenting interactions and outcomes.

Requirements

• Active, unrestricted Registered Nurse license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• At least 2 years of clinical nursing experience; 3 or more years is preferred.
• Strong clinical judgment, critical-thinking, problem-solving, verbal communication, and written communication skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Microsoft Word, Excel, Outlook, multiple computer systems, and electronic medical records.

Nice to have

• Experience caring for elderly or chronically ill populations.
• Experience in care coordination, case management, managed care, Medicare, or Medicaid.
• Experience in chronic-condition management, patient education, telephonic nursing, health coaching, or remote care management.
• Experience working with multidisciplinary healthcare teams.
