RN Field Care Management Team Lead (HCBS & LTSS)

Location: Michigan, specifically Wayne and Macomb Counties. Remote work with field responsibilities and 75%-90% travel within Wayne and/or Macomb Counties. Monday through Friday, 8:00 AM to 5:00 PM Eastern Time.

Salary: $71,100-$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead daily care management operations and partner with the Manager of Care Management to address barriers and improve team performance.
• Assign workloads for Care Coordinators, Community Health Workers, and CMSA 2 personnel based on caseload, referrals, and geographic needs.
• Conduct or support member assessments, onsite visits, problem resolution, and care coordination for adult populations.
• Coordinate interdisciplinary care team activities, nonclinical assessments, scheduling, and communication.
• Serve as a preceptor, trainer, and subject matter expert for clinical associates.
• Contribute to MI HIDE program strategy and operational improvement initiatives.

Requirements

• Active Michigan RN, NP, or CNS license in good standing with no disciplinary actions.
• Residence in Michigan, specifically Wayne or Macomb County.
• Prior care management experience involving clinical team leadership, mentoring, training, or support.
• Knowledge of HCBS, LTSS, Medicaid regulations, person-centered planning, community resources, and care coordination.
• Experience with in-home patient care, LTSS assessments, chronic disease management, and patient education.
• Proficiency with EHR/EMR systems, Microsoft Word, Outlook, Excel, web-based tools, and multidisciplinary collaboration.

Nice to have

• BSN and 3-5 years of experience in in-home assessment, care coordination, and case management.
• Healthcare lead or supervisory experience and managed care experience.
• Experience with InterRAI IHC Assessments and Nursing Facility Level of Care Determinations.
• Spanish, Arabic, or Chaldean Neo-Aramaic bilingual proficiency.
• CCM certification and Motivational Interviewing certification or experience.
