Behavioral Health Care Manager (ECM) – Licensed (Hybrid)

Location: Riverside, California, United States; hybrid schedule with 2–3 days in the field and 2–3 days in the office

Salary: $95,000–$100,000 per year

What you will do

• Serve as the primary contact for members with behavioral health and psychosocial needs.
• Conduct comprehensive assessments and develop person-centered care plans.
• Coordinate care among behavioral health providers, medical teams, and community resources.
• Conduct in-person home and community visits and support transitions of care.
• Provide post-discharge follow-up, education, coaching, and trauma-informed support.
• Maintain accurate documentation and meet ECM program requirements.

Requirements

• Master’s degree in Social Work or Psychology.
• Active license as a Clinical Social Worker, Marriage and Family Therapist, or Professional Clinical Counselor is required.
• At least one year of experience in care management, behavioral health, or a related setting; Medi-Cal or ECM experience is preferred.
• Strong communication and care coordination skills, with knowledge of behavioral health, social determinants of health, and community resources.
• Comfort with field work and in-person home visits.
• Valid driver’s license, insurance, and reliable vehicle.
