Registered Nurse Care Manager (RNCM) - Low Desert

Location: Palm Springs, California, United States; Low Desert and Coachella Valley field coverage, including Indio. Hybrid work with work-from-home options several days a week and required field-based member engagement as needed.

Salary: $95,000–$110,000 per year

What you will do

• Provide clinical oversight and guidance to non-clinical Lead Care Managers.
• Review member charts, care plans, medical histories, and treatment adherence in EMR/EHR systems.
• Identify gaps in care and recommend interventions for members with complex medical and social needs.
• Coordinate with multidisciplinary teams, healthcare providers, hospitals, and community organizations.
• Conduct field-based visits, in-person assessments, member engagement, and follow-up activities.
• Support hospital discharge planning, weekly case reviews, regulatory reporting, and documentation.

Requirements

• Active Registered Nurse license in California required.
• At least 2 years of nursing experience.
• At least 1 year of experience in care management, case management, or a leadership capacity.
• Experience supporting vulnerable populations and people with complex medical and social needs.
• Strong clinical assessment, care planning, communication, collaboration, and clinical judgment skills.
• Ability and willingness to perform field-based work and in-person member engagement as required.
