Field Community Health Worker (Wayne or Macomb County)

Location: Must reside in Michigan and travel within Wayne or Macomb Counties. Home-based role with regular field visits; travel requirements are 75%–90% within the assigned counties. Schedule: Monday through Friday, 8:00 AM–5:00 PM Eastern Time.

Salary: $41,900–$56,600 per year.

What you will do

• Engage Medicare and Medicaid members as part of care support teams.
• Conduct in-person assessments, home visits, provider visits, and community outreach.
• Address social determinants of health and coordinate access to housing, food, income, transportation, and healthcare services.
• Support chronic-condition self-management, treatment adherence, prevention, and health education.
• Advocate for members with providers and community organizations, including accompanying members to appointments when requested.
• Build relationships with community resources, identify resource gaps, and support hard-to-reach members.

Requirements

• Michigan residence and ability to travel within Wayne or Macomb Counties are required.
• At least 2 years of experience with community resources, health agencies, or social services.
• Intermediate proficiency with Microsoft Office, including Teams, Excel, PowerPoint, Outlook, and Word.
• Strong written communication skills, member advocacy, and respect for cultural and demographic diversity.
• Valid state driver’s license and personal vehicle liability insurance are required.
• Patient-facing work requires completion of hirify.global’s TB screening program.

Nice to have

• Community Health Worker training or certification.
• Bachelor’s degree in Social Work or a related field.
• Familiarity with state Medicaid program guidelines.
• Experience supporting Medicaid enrollees with physical or behavioral health needs and varied health literacy.
• Fluency in English, Arabic, and/or Chaldean Neo-Aramaic; language assessment required.
