Social Worker II (Chronic Care)

Location: Remote; travel up to 20% required

Target compensation: $55,000–$73,000 per year

What you will do

• Provide advanced psychosocial support to individuals and families living with complex chronic conditions.
• Assess medical, emotional, and socioeconomic needs and develop person-centered care plans.
• Coordinate care with interdisciplinary teams and community partners.
• Connect individuals with critical health and social resources.
• Support advocacy, education, and outreach across diverse and underserved populations.

Requirements

• Master’s degree in Social Work.
• Current clinical social worker licensure or eligibility for licensure.
• At least two years of social work experience, preferably in healthcare, chronic disease management, or nonprofit settings.
• Excellent written and verbal communication, organizational skills, and attention to detail.
• Ability to travel up to 20%.

Nice to have

• Experience supporting dialysis patients or individuals with end-stage renal disease.
