What you will do

• Review medication-related gaps in care for high-risk patients as part of a multidisciplinary team.
• Research claims data and clinical information to evaluate each patient's situation.
• Develop clinical recommendations to close identified gaps in care.
• Contact patients, caregivers, physicians, and pharmacists to coordinate interventions.
• Apply clinical judgment and discretion while supporting case and disease management programs.

Requirements

• Bachelor's degree in Pharmacy.
• Registered Pharmacist license is required.
• 5–7 years of pharmacy experience, or 1–2 years with completion of a clinical residency program.
• 3–5 years of experience in pharmacist utilization management or medical therapy management.
• Strong oral and written communication, telephone, problem-solving, decision-making, compliance, and collaborative problem-solving skills.
• Physical work site required; the position is performed remotely from Pennsylvania.
