CHOICES Care Coordinator (Wayne and Lawrence Counties)

Location: Remote within Wayne or Lawrence County, Tennessee, USA; the role includes travel to members' homes for face-to-face visits and requires availability from 8:00 a.m. to 5:00 p.m. EST.

What you will do

• Partner with members and families to identify supports and direct services for health, employment, independent living, and community participation.
• Collaborate with clinical and social support colleagues to address physical, behavioral health, and long-term service needs.
• Conduct objective face-to-face home and community visits and assess physical, behavioral, functional, psychosocial, financial, employment, and independent living needs.
• Use established criteria to authorize appropriate home- and community-based services.
• Confirm that authorized services are delivered and that members' needs are being met.

Requirements

• Must live in Wayne or Lawrence County, Tennessee, and be able to travel to members' homes.
• At least 2 years of clinical experience; the role description also references 3 years of clinical setting experience.
• Active Tennessee registered nurse license, or an active license in a Nurse Licensure Compact state; alternatively, a Master's in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license.
• Valid driver's license, appropriate auto insurance, and an acceptable motor vehicle record for routine business driving.
• Strong customer service, communication, interpersonal, organizational, time management, and prioritization skills.
• Basic Microsoft Office, email, and digital literacy skills.
