Case Manager - Eau Claire (Healthcare)

Location: Remote role serving Eau Claire and surrounding areas, Wisconsin, United States; up to 40% travel and recurring in-person member visits required. Work hours are Monday–Friday, 8:00 am–4:30 pm CST. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $53,700–$72,600 per year

What you will do

• Assess members’ health and safety needs, strengths, interests, and preferences to create comprehensive Member Care Plans.
• Coordinate with Field Care Nurses to deliver services in the least restrictive environment defined by each care plan.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at members’ residences.
• Complete quarterly in-person visits and maintain monthly telephone contact with members.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, educate members, ensure cost-effective service delivery, and maintain case documentation and service authorizations.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to serve members in Eau Claire and surrounding areas, including recurring in-person visits and up to 40% travel.
• Ability to work Monday–Friday, 8:00 am–4:30 pm CST.
• Ability to work from a dedicated home workspace and maintain internet speeds of at least 25 Mbps download and 10 Mbps upload while protecting member PHI and HIPAA information.
• Completion of tuberculosis screening is required for this patient-facing role.
