Social Worker

Location: Remote within the USA, Tennessee; flexible scheduling during business hours from 8:00 a.m. to 6:00 p.m. EST. Home visits may be required.

What you will do

• Provide telephonic outreach, crisis intervention, advocacy, and care coordination for elderly and disabled members.
• Assess patients' and families' ongoing medical, behavioral, social, and emotional needs.
• Plan interventions that support psychological, emotional, and physical recovery.
• Coordinate post-discharge care with external agencies and home care nursing groups.
• Connect members and families with healthcare, community, and social service resources.
• Conduct home visits when necessary.

Requirements

• Active, unrestricted Tennessee LCSW or LMSW license required.
• Clinical licensure or a master's degree in education required.
• At least 3 years of experience providing care coordination for people receiving long-term care or home and community-based services.
• Valid driver's license required; vehicle use requires compliance with applicable insurance and driver safety requirements.
• Strong communication, organization, strategic thinking, creativity, attention to detail, and ability to work with ambiguity.
• Awareness of cultural and socioeconomic factors affecting healthcare and mental healthcare access.

Nice to have

• Experience in skilled nursing facilities, acute inpatient settings, discharge planning, community resource coordination, or hospice care.
• CCM certification.
