RN Care Manager

Location: Milwaukee, Wisconsin; remote from home with approximately 40% travel for in-person member visits. Must hold a Wisconsin Registered Nurse license and reside within 45 minutes of the assigned coverage area. Monday–Friday, 8:00 am–4:30 pm CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and coordinate with acute and primary care providers.
• Educate members about disease processes, risks, nutrition, exercise, and lifestyle modifications.
• Conduct monthly phone contacts and travel to members’ settings for in-person visits.
• Review records, identify remediation needs, complete documentation, and assign follow-up tasks in D365.
• Support newly assigned RNs and Care Coach partners and accept complex member assignments.

Requirements

• Associate degree in nursing.
• Active Registered Nurse license in Wisconsin in good standing.
• Valid state driver’s license and personal vehicle liability insurance with 100/300/100 limits.
• Ability to travel approximately 40% for member visits and other work-related activities.
• Dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload speeds.
• Ability to complete required TB screening for this patient-facing role.
