RN Care Manager (Healthcare)

Location: Remote from the Eau Claire, Wisconsin surrounding coverage area; travel up to 40% for in-person member visits and occasional hirify.global office training or meetings. Candidates should reside within 45 minutes of the assigned coverage area and work Monday through Friday, 8:00 am–4:30 pm CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and coordinate with acute and primary care providers.
• Educate members on disease processes, nutrition, exercise, lifestyle changes, and related risks.
• Maintain member documentation within required Wisconsin DHS contract timeframes.
• Conduct monthly phone contacts and travel to members’ settings for in-person visits.
• Review and remediate records, assign follow-up tasks in D365, and support newly assigned RNs and Care Coach partners.

Requirements

• Associate degree in nursing.
• Registered Nurse license in Wisconsin in good standing.
• Residence within 45 minutes of the assigned Eau Claire-area coverage area.
• Valid state driver’s license and personal vehicle liability insurance with 100/300/100 limits.
• Ability to travel up to 40% for member visits.
• Patient-facing work requires TB screening; a dedicated home workspace and internet service with at least 25 Mbps download and 10 Mbps upload are required.

Nice to have

• Three years of care management experience.
• Previous care management experience.
