Care Management Support Professional 1 - SDOH

Location: Remote, must reside in Oklahoma; occasional travel to hirify.global offices for training or meetings may be required.

Salary: $45,400–$61,300 per year

What you will do

• Find community-based resources addressing housing, transportation, food insecurity, education, employment, and other social determinants of health.
• Support Care Managers and Community Health Workers with housing resources and additional enrollee needs.
• Coordinate referrals among hirify.global care managers, healthcare providers, enrollees, and community organizations.
• Handle resource-assistance calls, support health screenings and surveys, and refer enrollees to Care Management when appropriate.
• Monitor care-plan compliance, provide motivational interviewing, and deliver culturally appropriate health education and coaching.
• Conduct community assessments, maintain provider and community-resource relationships, and attend community events.

Requirements

• Must reside in Oklahoma.
• At least 1 year of experience in community outreach, community engagement, public health, community health, or healthcare.
• 3 years of experience with Microsoft Word, Excel, and Outlook.
• Ability to make decisions regarding work priorities, assignments, standards, and resources.
• Dedicated, interruption-free workspace to protect member PHI and HIPAA information.
• Home internet with at least 25 Mbps download and 10 Mbps upload speeds.

• Bachelor's degree.

• Microsoft Access experience.
