Community Care Advocate

Location: Onsite in Chicago, Illinois, United States, serving zip codes 60640 and 60608. Schedule: Monday 9:00 AM–5:30 PM; Tuesday 8:30 AM–5:00 PM; Wednesday–Thursday 8:00 AM–5:30 PM; Friday 9:30 AM–6:00 PM.

Salary: $28–$33 per hour

What you will do

• Connect employees with work-life, financial, legal, housing, food, transportation, and other basic-needs resources.
• Provide one-on-one consultations, referrals, application assistance, and follow-up support.
• Help employees navigate mental health benefits and connect with appropriate treatment options and community services.
• Coordinate with customer care teams and local contacts on emergency funding, resource referrals, and employee needs.
• Document referrals, demographics, outcomes, connection rates, and other program metrics.
• Promote the service, attend customer meetings, maintain partner communications, and submit timely activity reports.

Requirements

• Bachelor’s degree in a related field
• Proficiency in Spanish required
• Experience working with diverse populations in social work or related settings.
• Experience connecting individuals with local and national resources.
• Ability to balance multiple stakeholders and communicate effectively in writing and verbally.
• Comfort using multiple computer platforms in a technology-enabled environment.
