Field Nurse Practitioner / Physician Assistant

Location: Detroit, Michigan, United States. Workplace: On-site, with daily travel within approximately a 55-mile radius of the assigned county.

Compensation: Approximately $100 per completed assessment, plus paid mileage.

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform focused histories, physical examinations, vital sign checks, medication reviews, and preventive screenings.
• Complete screenings such as Hemoglobin A1C, spirometry, KED, DRE, and FIT.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Work independently within the NP or PA scope of practice and applicable state regulations.

Requirements

• Active, unencumbered Nurse Practitioner or Physician Assistant license in the applicable state.
• Ability to comply with state-specific supervision or collaboration requirements, where applicable.
• At least three years of patient care experience preferred.
• Comfort with EMR systems and independent field-based work.
• Ongoing availability of approximately 30 hours per month and reliability in honoring confirmed visits.
• Willingness to obtain additional licensure if needed, with support from hirify.global.
