LTSS Service Care Coordinator (Healthcare): Coordinating holistic long-term care plans and services for members in Texas with an accent on foster care, medically fragile children and adolescents, and access to community resources. Focus on assessing member needs, arranging provider services, conducting face-to-face assessments, and monitoring care outcomes and regulatory compliance.

Location: Remote in Texas; must reside in the state of Texas. The role includes 25% travel for face-to-face assessments and may require on-site member visits.

Salary: $22.94–$38.79 per hour

What you will do

  • Assess member needs, available resources, and appropriate long-term care services.
  • Develop and monitor personalized care plans for long-term care members.
  • Coordinate services among members, families or caregivers, healthcare providers, specialists, and community resources.
  • Educate members and caregivers about healthcare benefits, service options, referrals, and provider instructions.
  • Conduct face-to-face or on-site assessments and document member information and care management activities.
  • Provide recommendations to improve quality of care and cost-effective service delivery.

Requirements

  • Must reside in Texas.
  • Bachelor’s degree and at least one year of related experience, or equivalent applicable experience.
  • Recent direct experience working with people with disabilities or vulnerable populations with chronic or complex conditions, including children and young adults.
  • Previous case management experience in foster care is preferred for the role.
  • Experience with medically fragile children or adolescents is preferred.
  • Ability to travel 25% for face-to-face assessments and perform occasional on-site visits.
