RN Clinical Care Coordinator - Hospitalist - EMG - Full Time

What you will do

• Manage a panel of active patients requiring complex care management and coordination.
• Design and implement individualized care plans with patients, families, caregivers, and multidisciplinary providers.
• Perform short-term transitional care and longer-term planning across the continuum of care.
• Provide chronic disease self-management education, health coaching, motivational interviewing, and support with medications and advanced care planning.
• Monitor adherence and progress toward care goals, updating action plans as needed.
• Coordinate access to medical, specialty, community, financial assistance, and other support resources.

Requirements

• Graduate of an accredited nursing program.
• Current Registered Nurse licensure in New Hampshire required.
• Registered Nurse licensure in Massachusetts is preferred.
• Five or more years of nursing experience in acute, skilled nursing, community health, and/or ambulatory settings required.
• Three or more years of care coordination or case management experience preferred.
• Specialty certifications and experience with ACOs or value-based contracts are preferred.
