Registered Nurse (RN) - Clinical Care Coordinator

Location: Onsite in Charlottesville, Virginia, United States; Monday–Friday schedule with no nights, weekends, holidays, or on-call.

Salary: $90,000–$105,000 per year, based on experience.

What you will do

• Coordinate care and advocate for patients with chronic conditions.
• Assess patients and develop individualized care plans.
• Educate and coach patients to improve chronic-condition self-management.
• Lead interdisciplinary care team meetings and collaborate with providers.
• Identify high-risk patients using population health tools.
• Document care plans, support referrals and transitions, and contribute to quality improvement initiatives.

Requirements

• Bachelor’s degree in Nursing.
• Active RN license in good standing; a multistate license is preferred.
• Current CPR certification.
• Five or more years of nursing experience.
• Strong communication, organization, and critical-thinking skills.
• Experience with EMRs and Microsoft Office.

Nice to have

• Experience in care coordination, case management, chronic disease management, or population health.
• Experience with motivational interviewing.
