Occupational Therapist

Location: 702 North Azusa Avenue, West Covina, California, United States. The role includes home visits and travel between work sites or community locations as needed.

Salary: $107,000–$123,000 annual base salary, plus a target annual bonus of 5%.

What you will do

• Provide occupational therapy evaluation and treatment across acute, restorative, and maintenance levels of care.
• Assess participants, create individualized treatment plans, and coordinate care with the Interdisciplinary Team.
• Conduct home visits and educate participants, caregivers, and staff on ADLs, exercise programs, safety, and DME use.
• Document therapy services, participant responses, care plans, and department records in accordance with regulatory requirements.
• Support DME ordering, inventory, distribution, and maintenance.
• Contribute to quality assurance, infection control, staff education, data collection, and program standardization.

Requirements

• Master’s or Doctorate in Occupational Therapy, or an equivalent degree from an accredited program meeting AOTA and ACOTE standards.
• National certification by the American Occupational Therapy Association or eligibility for its national registration examination.
• At least one year of experience working with frail or elderly populations.
• Ability to work in participant care settings, travel between work sites, and perform physical duties including lifting or moving supplies and equipment up to 25–50 pounds with assistance or mechanical aids.
• Employment requires authorization to work in the United States; hirify.global participates in E-Verify.
• Ability to adapt in a fast-paced environment, seek feedback, use data, and contribute to continuous improvement.

Nice to have

• Two or more years of practicing occupational therapy experience.
• Spanish/English bilingual ability; other languages are also valued.
• PACE or ADCH/CBAS experience.
• State-issued driver’s license, personal transportation, and auto insurance as required by law.
