Manager, Care Coaching (Healthcare)

Location: Remote Florida; must reside within 3 hours of Broward County. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $78,400–$107,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead and support a team of 15–17 Care Coaches.
• Maintain team compliance, coaching, accountability, and caseload ratio standards.
• Use assessments, data, member conversations, and active care planning to monitor patient progress.
• Analyze metric-driven reports and identify barriers and support strategies.
• Minimize mileage and overtime while maintaining effective care delivery.
• Collaborate with departments and managers, and conduct briefings and area meetings.

Requirements

• Bachelor’s degree in a health-related field.
• At least 2 years of managed care experience in Medicare and/or Medicaid.
• At least 1 year of management or leadership experience and 1 year of complex membership experience.
• Experience analyzing and interpreting trends, leading teams, and working in the health solutions industry.
• Valid state driver’s license and personal vehicle liability insurance with limits of at least 100,000/300,000/100,000.
• Florida-based applicants are subject to Florida Level 2 background screening.

Nice to have

• Current RN or LPN state licensure.
• Master’s degree in human services, social sciences, or a health-related field.
• Experience in utilization management, discharge planning, home health, rehabilitation, health promotion, coaching, or wellness.
• Health Plan, Medicare/Medicaid, financial trend analysis, or NCQA standards experience.
